Registration of a child's medical record
Having received from the district pediatrician a thin booklet of A4 size, the child will have to undergo a medical examination from the specialized specialists. Therefore, once the 026 form is in the hands of the parents, it is better not to hesitate and immediately go to the polyclinic registry and make an appointment with: ENT, oculist, dermatologist, surgeon, dentist, neurologist and orthopedist. Each of the listed specialists will examine the crumbs and give an opinion about the state of his health, put a date and a signature. However, adults should be prepared to fill out the child's medical card (form 026 y) not one day, as the hours and days of admission for all doctors are different. Also in the calculation it is necessary to take the big turns and unforeseen situations (such as vacation or hospital or something else of that kind), which now and then happen at the most inopportune moment.
After circumvention of the doctors, the baby will have to pass tests, the directions for which are usually attached to the 026 form. As a rule, the preschooler takes: a clinical blood test, a general urine test, and feces and scraping for the eggs of the worm and enterobiasis.
If the parents managed to do everything necessary for a week, we can say that they were very fortunate. But unfortunately, this does not end there. Having received the conclusions of narrow specialists and having passed the necessary tests, the mother and the baby again go to the pediatrician. He conducts a follow-up inspection, measures the height and weight of the crumbs, and also compulsorily introduces information about the vaccinations that have been made to date and the history of the diseases transferred. The completed card is given for signature to the head physician, after which it can be considered an official document.
It should be noted that in addition to all of the above, the medical card must contain information about the parents, place of residence or residence registration, and of course the last name, first name, patronymic of the child (it is necessary to check the spelling) and the date of his birth.
