Bisphosphonate preparations

A special class of drugs that prevent bone loss and its destruction by osteoclasts is actively used in the treatment of osteoporosis in women. Bisphosphonate or dysphosphonate preparations are synthetic compounds that are structurally very similar to natural pyrophosphates resistant to resorption. Today they are considered the only medications for osteoporosis with proven effectiveness.

Names of preparations of bisphosphonate group

The type of medication considered is classified into 2 large groups - drugs containing nitrogen and drugs without it.

The first variety includes:

  1. Alendronic acid. Intensively stimulates the restoration of the structure and the formation of the correct histological structure of bones, regulates the exchange and resorption processes. It is usually prescribed for senile and post-menopausal osteoporosis, malignant hypercalcemia and deforming osteitis, to prevent fractures;
  2. Zolendronate or zoledronic acid. Suppresses the activity of osteoclasts, but does not affect the processes of bone mineralization, mechanical properties and tissue formation;
  3. Clodronic acid (Clodron, Bonefos). Reduces the risk of fracture of the bones, produces an anesthetic effect. It is used in experimental medicine, selectively destroys pathological macrophages;
  4. Bondronate (ibadronic acid). Designed specifically for the treatment of women, especially relevant in the climacteric period. Also used during hormonal replacement therapy.

Bezazotistye bisphosphonates can be taken with metastases in the bone, severe forms of malignant tumors, hypercalcemia. In this case, the correct choice is the correct dosage, which the doctor must calculate. Otherwise, especially with an overdose, complications may occur.

List of bisphosphonate preparations without nitrogen:

  1. Tiludronate. Increases the density of bone tissue, so it is often prescribed in the presence of deformities and fractures;
  2. Xidiphon, Pleistat or sodium etidronate. It is widely used in complex therapy of Paget's disease, oncological diseases, hypercalcemia, severe osteoporosis;
  3. Ibandronate sodium. It helps well with hormonal drugs for substitution treatment in the post-menopausal period;
  4. Clodronate. Prevents the destruction of calcium crystals, the development of osteolysis. The drug is prescribed for malignant tumor tumors, leukemias, lymphomas, extensive metastases.

Instruction for bisphosphonates

Admission of the medicines described above is carried out in accordance with the doctor's recommendations 1 time per day.

Bisphosphonates are very poorly soluble, so they need to be washed down with extremely pure boiled water at room temperature for best absorption.

It is desirable to observe a break between admission food and bisphosphonate drugs. Tablets should be taken 1,5 hours before meals on an empty stomach - no earlier than 60 minutes after a meal.

One of the unpleasant features of this group of drugs is their ability to irritate the mucosa of the esophagus, to provoke the formation of small ulcers on its surface. Therefore, after taking bisphosphonates you can not go to bed immediately, it is important for 90 minutes (minimum) to be in an upright position, you can sit, but it's better to walk on foot or just do housework. This will prevent such side effects as heartburn , reverse reflux and esophagitis.
