How does the pregnant exchange look like?
In general, the exchange card can be conditionally divided into 3 parts. The first of them is called "Information of a woman's consultation about a pregnant woman". Here the doctor of the women's consultation in details describes the information about the course of previous pregnancies, about childbirth, the postpartum period. This knowledge plays a big role for the doctor, the lead child, as well as for the pediatrician in the maternity hospital. Filling of this information is carried out at the first visit of a woman to a woman's consultation with a new pregnancy.
At all subsequent visits to the antenatal clinic, the pregnant woman must bring an exchange card with her, so that the doctor can make notes about her on current examinations and studies.
If it happened that a woman went to the hospital for support and treatment, as well as for childbirth, a woman must present her exchange card. If she lost the exchange card or forgot her, the woman is placed in the 2nd obstetrical ward, where all women who did not undergo the necessary examinations, as well as pregnant women with the detected infections, arrive so that they do not infect other patients.
If a woman was hospitalized in the department of pathology before the time of issue of the exchange card (22-23 weeks), she must give out the exchange card ahead of time and record the results of the available tests and examinations.
In the second ticket, entitled "Information of the maternity hospital, the maternity ward of the puerpera", records are made in the maternity hospital before the woman's discharge. He is issued to her for presentation to a women's consultation. At filling of this coupon, the doctor in detail writes down
And, at last, the third coupon - "Information of the maternity hospital, the maternity ward of the hospital about the newborn." It is filled in the children's ward of the maternity hospital just before the mother and newborn is discharged and is given to the mother of the child for transfer to a children's polyclinic.
When filling out the third coupon, obstetric hospital doctors describe the features of the birth course, the state of the newborn, indicate the need for special monitoring, if any.
